This Client Agreement and Waiver (the "Agreement") is entered into between Joy Core Concection and the undersigned client ("Client").

Terms and Conditions:
Services: By checking the box that says "I agree to these terms and conditions," the Client acknowledges that they have read, understood, and agreed to the terms and conditions outlined in this Agreement. This Agreement governs the provision of services by Joy Core Connection, including but not limited to yoga therapy and health coaching.
Payment: The Client agrees to make payment for services as outlined in the payment agreement separately provided. Payment terms and schedules are subject to the terms of that agreement.
Cancellation Policy: Client agrees to abide by our cancellation policy, which is detailed in our separate cancellation policy document.
Confidentiality: All client information and discussions will be treated with the utmost confidentiality, as outlined in our Privacy Policy.
Intake Health Questionnaire: The client agrees to complete the form fully and accurately and provide updates in the pre-session forms provided prior to the session.
Health and Safety: The Client understands that participation in any health or fitness program carries inherent risks, and it is their responsibility to consult with a healthcare provider before beginning any new program. Joy Core Connection will not be held responsible for any injuries or health issues that may arise during or after participation in our programs.
Client Commitment: The Client is committed to actively participate in the program and follow any recommendations provided by Joy Core Connection for their health and well-being.
Termination: Joy Core Connection reserves the right to terminate this agreement and services provided if the Client engages in behavior that is disruptive, non-compliant, or unsafe, or for any other valid reason at the discretion of Joy Core Connection.
By checking the box that says "I agree to these terms and conditions," the Client confirms their understanding and acceptance of this Agreement.
